A reliable care record
Consultations record relevant findings, decisions, medicines, investigations, advice and the agreed review point.
Membership supports a consistent clinical record, clearer follow-up and communication with the teams involved in your care.
Current fees, included access, additional charges and membership terms are provided in writing before enrolment. Please do not email medical details.

Membership should make access clearer without weakening clinical judgement, confidentiality or the need to use urgent services appropriately.
Consultations record relevant findings, decisions, medicines, investigations, advice and the agreed review point.
You should know who is arranging each next step and when Allhours GP’s responsibility ends or passes to another service.
Relevant information can be shared with consent to support care, with additional disclosure where safety or law requires it.
The plan organises access. The doctor still makes each clinical decision according to your needs, the evidence available and the safest care setting.
Your written plan defines the commercial service. Your clinician defines the medical plan.
Accurate history, medicine lists, allergies, results and letters help the clinician understand what has already happened.
The doctor records clinical findings, your preferences, decisions and safety-net advice in a clear, contemporaneous record.
Referrals, tests, prescriptions and handovers identify which clinician or team is responsible and what information they need.
Results, response to treatment and new symptoms are reviewed at the interval set by clinical need and your plan.
Keep the plan summary and terms. Accurate member details and clear limits make care safer and prevent unexpected charges.
The member journey stays clear from the first plan summary through each consultation and follow-up.
The consultation captures the information and decision needed for safe ongoing care.
You receive the plan, warning signs, responsibilities and review timing.
Relevant information is sent to another clinician with consent or where otherwise justified.
Results and progress are considered by the responsible clinician or service.
The practical details should be clear before you enrol or rely on membership for care.
Not automatically in every case. The clinician considers what is relevant for safe ongoing care and discusses consent, communication and any reason information needs to be shared.
That decision belongs to your NHS GP and local arrangements. Do not assume that an NHS clinician will prescribe or monitor a treatment started privately.
The service may support continuity with a familiar clinician where practical, but safe availability sometimes requires another appropriately qualified doctor.
The plan should name the clinician or service responsible, expected timing and what to do if you have not heard. Membership does not replace that explicit result plan.
You can ask how to access your records and correct factual inaccuracies. Access is handled through the service’s secure information process.
Email your name, contact details and whether you are interested in Individual or Family membership. Please do not include symptoms, medicines or other medical information in ordinary email; the team will explain the secure route when clinical information is needed.
