Choose a service

Five focused routes.

Choose the document you have been asked to provide. Each page explains what to send first, whether an appointment is needed and what can delay completion.

Not sure which report you need? Ask the receiving organisation for its current form and written requirements before you contact us. A generic certificate may not be accepted.

A simpler process

Checked before you book.

Medical forms are not interchangeable. We review the request first, confirm whether a GP can complete it and tell you exactly what evidence to bring.

The clinician may need more information, tests or specialist input. The report is completed only when there is enough evidence to support it.

01

Get the current form

Ask DVLA, your council, employer, insurer or specialist provider for the latest version and instructions.

02

Let us check it

Email the document type and official link or blank form. We will arrange a secure route for health information.

03

Confirm the appointment

We explain the right format, evidence, fee and appointment length before you book.

04

Complete what is supported

The doctor reviews the evidence and examination, then completes the report or referral when appropriate.

Prepare once

The right information saves time.

Exact requirements differ, but most report appointments need a clear request, proof of identity and reliable clinical evidence.

Usually needed

  • The latest form or written questions
  • Instructions from the receiving organisation
  • Photo identification
  • A current medication list
  • Relevant records, results or specialist letters

Always worth checking

  • Who is allowed to complete the form
  • Whether an in-person examination is required
  • Whether an optician, specialist or occupational physician is needed
  • The deadline and delivery method
  • Any insurer or employer authorisation
Our approach

Accurate, proportionate, independent.

A premium service is not a promise to sign every form. It is a careful process that respects your information and gives the receiving organisation a reliable document.

01 / Scope

Right clinician, right request

We confirm that a GP is the appropriate professional and has access to enough information before accepting the work.

02 / Privacy

Only what is relevant

Reports are limited to the authorised purpose and sent through an appropriate route to the intended recipient.

03 / Judgement

No automatic certification

The doctor records an independent opinion. DVLA, a council, employer, insurer or specialist provider makes the final decision.

Good to know

Common questions.

A little preparation makes the process much more predictable.

Can I book before I have the form?

We recommend waiting until you have the receiving organisation’s current form or written questions. It lets us confirm the right clinician, appointment and evidence before you pay.

How much does a report cost?

The fee depends on the form, record review, examination, tests and time needed. We confirm the full fee before booking after we have checked the request.

Will the document be completed at the appointment?

Sometimes. It depends on the form and whether all evidence is available. Missing records, an incomplete vision section, abnormal findings or further questions can mean more information is needed first.

Can you guarantee that I will be declared fit or referred?

No. Doctors must use independent clinical judgement. A certificate, fitness opinion or referral is provided only when supported; the receiving organisation makes its own final decision.

Can I send my medical records by email?

Please do not send medical details to the general inbox. Start with the report type, official form or link and your contact details. We will explain the secure route if records are needed.

Request an appointment

Let us check the request first.

Email the report type, the organisation requesting it and your contact details. Please do not include medical information in ordinary email. We will reply with the secure next step and confirm suitability, what to bring, the fee and appointment length before booking.
